Melasma and Pigmentation: A Practical Guide to Safe Treatment

Melasma is one of the hardest pigmentation conditions to treat, and the biggest reason patients get worse instead of better is aggressive DIY treatment. Harsh scrubs, unregulated bleaching creams and unlabelled peels almost always backfire.

Why melasma is different

Unlike sun-tan or post-acne marks, melasma is driven by hormones as well as UV exposure. That means it can return even after successful treatment if sun protection slips. Think of treatment as a marathon, not a sprint.

What actually works

  • Daily broad-spectrum SPF 50+ — non-negotiable. Reapply every 3 hours if you are outdoors.
  • Topical brighteners — tranexamic acid, azelaic acid, kojic acid, and short cycles of prescription hydroquinone under supervision.
  • Chemical peels — mild lactic or mandelic peels done every 3–4 weeks, only by a qualified dermatologist.
  • Laser toning — only for stubborn cases, and never as a first-line treatment.

What to avoid

Please do not use anonymous fairness creams from social media. Steroid-based mixes give a quick lightening effect but almost always cause worse pigmentation, thin skin and red patches within 3–6 months.
